Call the hospital medical records department and ask them to email you a consent to release your medical records. When you fill it out, check that you want ALL OF THEM.. you will have to pay a fee and it may take a week or so to get them. Some hospitals require you pick them up in person. They may waive this if you send a photocopy of your ID with your consent to release all records.  Hospitals historically have purged these after 10 years. Not sure nowadays with the new digital age we live in.You have a right to send in corrections to thiings you know are incorrect. Good luck</description>

		<description>I feel my records will highlight the curiousness that I presented to St Joseph ER  on 12/3/05 and needed 2 stents. Then the identical feelings brought me back into St Josepth ER on 12/27/05. I have ny stent cards.
It was in my MidAtlantic records, a paragraph about &quot;either they didn&#039;t look high enough the first time: (my concern) or the progression of my disease was much more significant than they initially assessed. ???
I have now moved to Ohio 12/07-   01/08     and am still here. I have had three heart attacks total , two with Midei&#039;s practice, and one additional blockage with their practice in like maybe 4/06. Please see what you can do to obtain my records. They wanted me to have open heart the second time but I was petrified and refused. I now have 6 stents and much less function.   Am appealing disability.</description>
