Cardinal Health has issued a nationwide Alaris Infusion Pump recall for model 8100, which was previously known as the Medley Pump. The hospital infusion pumps are used to deliver controlled amounts of medication or other fluid. Due to manufacturing defects, the devices could deliver too much of the prescribed fluid, potentially resulting in a serious or fatal injury.

In November 2007, the U.S. Centers for Medicare and Medicaid Services (CMS) released a list of the 54 worst nursing homes in America. The list of the poorest performing homes was pulled from a complete report containing 128 “special focus facilities,” which are subject to more frequent inspections and scrutiny by regulators due to their history of bad care. However, the government has refused to release the full list despite criticism from nursing home advocates and several U.S. Senators.

Last month the FDA issued a second warning about Duragesic patch dangers which have claimed the lives of many people who never should have been given the powerful narcotic painkiller. The consumer protection group Public Citizen has filed a federal lawsuit against the FDA regarding tendon problems associated with fluoroquinolone antibiotics. They are asking the court to require the federal drug regulators to act on a petition filed 16 months ago requesting that new warnings be added about side effects of antibiotics, such as Cipro and Levaquin, which could increase the risk of tendonitis and tendon ruptures.

Last month a nationwide heparin syringe recall was issued by AM2 PAT, Inc. for their Sierra pre-filled lock flush solutions. The heparin IV flush syringes were found to be contaminated with Serratia marcescens, which could cause serious and potentially life threatening infections. A recent study indicates that Lipitor side effects may increase the risk of a brain hemorrhage when used after a stroke.
